We are offering an opportunity for a registered nurse, AHP, or social worker to join our multi-disciplinary team. We want to hear from you if you are highly committed to working with adults without a learning disability who have a diagnosis of autism.
The posts will either be based at Willis House or Rathbone Hospital, depending on what area staff will be working across.
Main duties of the job
The post holder will work as part of our team and would be responsible for supporting within our new provision, our Autism Intensive Support Team. You will provide person-centred support and recommendations whilst working alongside our mental health colleagues to best support engagement for autistic people with these mental health services. Training and ongoing continuous professional development will be available from a supportive and established team.

Detailed job description and main responsibilities
1. To have responsibility for a defined caseload of clients under the supervision of the Clinical Lead.
2. Assist with the appropriate screening of referrals and provide information for referrers, service users, and carers.
3. To support mental health professionals offer the best care for individuals suffering from difficulties relating to mental health.
4. To attend MDT meetings and offer professional oversight and input.
5. To clearly document findings and ensure ability to share with other professionals.
6. To liaise regularly with line manager providing them with an update of caseload.
7. Ensure intervention is provided in a range of settings suitable to meet the needs of the clients.
8. Work in a professional and collaborative manner with the multi-disciplinary team.
9. Provide information, education, and support for family members, carers, and other relevant professionals.
10. Participate and promote positive intra-agency working.
11. Participate in case load supervision to monitor the standards of service user case and ensure record keeping is in accordance with Professional and Trust standards.
12. Participate in regular clinical and managerial supervision with Clinical Lead/Team Manager, as per Trust policy.
13. Contribute to the development and implementation of integrated care pathways, including appropriate service user assessments/referrals and models of care.
14. Compile and maintain effective client records and record outcome measures in conjunction with clients and in accordance with Professional and Trust policies, procedures, and standards.
15. Participate in the induction, training or education of a variety of staff, students or volunteers attached to the team/service.
16. Participate and assist in setting standards of service user care based on sound research findings and evidence-based practice.

QUALIFICATIONS
* Professional Qualification – Nursing or AHP Up-to-date professional registration (to be maintained)
