Orchard Surgery Dereham are looking to recruit an experienced Visiting Nurse Practitioner/ Emergency Care Practitioner to work three/four days.
Main duties of the job
The role will be based at the practice having clinics and visiting patients living in care homes and their own homes within the locality.
You will have a Nurse Practitioner or Emergency Care Practitioner qualification and a prescriber is desirable but not essential.
The Successful candidate should be able to demonstrate high standards of clinical care, sound decision making and excellent interpersonal skills.
Salary is negotiable depending upon qualifications and experience

Job Summary
The rolewill be based across multiple sites focussing on the oversight of patientsliving in care homes and their own homes in the locality.
The postholder will be responsible for supporting the GPs with the management of frailand elderly patients, including first contact interventions. The post holder will take a comprehensivehistory, clinical assessment, treatment and evaluation of patients care. Theywill demonstrate safe clinical decision-making, ensuring that they act withintheir professional boundaries at all times. The post holder will workcollaboratively with the multi-disciplinary practice teams and the widerIntegrated Care team to meet the needs of patients and help to avoidunnecessary admissions to hospital. The post holder will provide support toconduct clinical audit, research, the development of clinical guidelines,procedures and protocols.
Key Responsibilities
ClinicalPractice
Providedirect consultation to patients in their own home, using decision making andproblem solving skills to make differential diagnoses and ensure clinicalpractice is effective, evidence based and to a high standard.
Agreemanagement plans, in liaison with families, in accordance with agreed practice,local and national guidance.
Demonstratesknowledge of the patho-physiology of acute and chronic conditions commonly seenin frail elderly patients.
Initiatesappropriate diagnostic requests and therapeutic interventions, with attentionto safety, cost, invasiveness, accessibility and efficacy.
Evaluatesthe patients condition and response to therapeutic interventions and modifiesthe plan of care accordingly.
As anindependent prescriber, prescribe safe, effective and appropriate medication foracute presentations, in accordance with current legislative framework.
Help todevelop clinical standards through policies and protocols that demonstrate evidencebased practice and respond to patient needs.
Ensure accuratecontemporaneous clinical records are maintained on the clinical systems,including the use of templates and alerts where necessary.
NurseManagement and Practice Administration
Act as apoint of liaison with other nurses across community services and the practice;act as mentor and clinical supervisor, supporting CPD.
Plan andprioritise own clinical workload.
Ensureadherence to the Drug Formulary, including the wound care formulary.
Participatein Practice meetings, reporting progress as required and liaising with othermembers of the local ICO team.
Health andSafety
Ensurepolicies are understood and observed and that procedures are followed.
Ensure thatappropriate use of equipment and facilities and ensure the environment ismaintained in good order.
Take thenecessary precautions to safeguard the welfare and safety of oneself and othersin accordance with the Health and Safety Legislation.

Undertakeappropriate training as the Partners see fit.
Person Specification
Qualifications
* BSc (Hons) Nurse Practitioner (Primary Health Care)
Experience
* Minimum of 3 years past experience in General Practice
* Can demonstrate competency in autonomous practice
* Demonstrates ability in teaching, research, audit and management
* Evidence of completed clinical audit
* IT Literate
* Up to date knowledge of national and local policies
* Ability to work effectively in a team and independently
* Experience of setting customer care and patient participation as a priority
