Job overview
The Older Adults Community Mental Health Team in Trafford is an integrated health and social care service that strives to provide quality recovery-based support to people with organic and functional mental health problems. Under the line management and clinical supervision from within the team, the post holder will maintain and co-ordinate timely and appropriate interventions for a caseload of service users living in the community within the framework of CPA, in line with relevant clinical pathways as outlined within the Older Adults CMHT operational procedure.

This includes respecting the individuality, values, culture, and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.

Detailed job description and main responsibilities
* Pay Enhancements – 30% additional for Evenings (8pm onwards) and Saturdays and 60% additional for Sundays and Bank holidays.
* 27 days annual leave plus bank holidays rising to 29 after 5 years and 33 days after 10 years.
* Knowledge of the needs of Older Adult Service Users.
* Knowledge of the needs of Carers.
Qualifications
* Registered RMN.
* Further CPD/training in mental health skills etc.
* Mentorship of students.
Experience
* Experience working with Older Adults.
* Experience working in a Community Mental Health Team.
* Experience in care planning and risk assessment.
* Experience supervising others.
* Experience working within an MDT.
* Experience working with a team, as well as autonomously.
* Passion for working with Older Adults and their Carers.
