Role Overview
We are excited to announce opportunities for 2 passionate and empathetic Senior Social Workers to join our dynamic Countywide Hospital Discharge Mental Health team. In this role, you will creatively support individuals to achieve their goals and live as independently as possible.
You will work with hospitals in Blackburn, Blackpool, Chorley, Lancaster, Ormskirk, and Preston. We strive to assign working locations close to your residence area whenever possible.
If you are a Senior Social Worker with a current AMHP qualification or are eager to pursue this through our fully funded development route, this is the perfect opportunity for you to make a meaningful difference.
The Role
Our fast-paced Mental Health Social Care Hospital Discharge Team requires a proactive leader to guide our Social Workers through challenging cases and daily tasks. This role involves working with adults with a range of mental health difficulties who require statutory interventions. You will undertake complex assessments, commission care, undertake duty tasks, and work as part of a multi-disciplinary team with a focus on collaboration and co-production.
You will support Lancashire residents being discharged safely and timely from the hospital wards. You will also assist the Team Manager with complaint handling, recruitment, supervision, and delivering Best Practice sessions to promote continued development.
We offer a range of development opportunities and support progression, including the opportunity to train as a Practice Educator. There is an expectation that Senior Social Workers will undertake their Approved Mental Health Professional qualification if not already held.
