Admission Avoidance Team
We are seeking to appoint a dynamic, confident, innovative and flexible Nurse or AHP with excellent communication and organisational skills. The post is 37.5hours per week and can be offered as either a permanent position or secondment opportunity.
This post holder will join our existing team of high functioning, highly motivated Advanced Clinical Practitioners, who deliver a wide variety of services including 2 hour Urgent Community Response, Hospital at Home/Virtual Ward, Falls pick up services and dedicated Care Home support, all enable people to be cared for in their usual place of residence. The Advanced Clinical Practitioners operate seven days a week between 08.00 to 20.00.
The post holder will work with partners system wide to provide integrated care across the health economy. The applicant will have qualified and hold an MSC in Advanced Clinical Practice.
The applicant must be a driver with access to a vehicle for work purposes subject to the provisions of the Equality Act

New Health and Social Care Act Regulations were approved by Parliament on the 22nd July 2021, which requires all those working or entering a CQC registered care home (for persons who require nursing or personal care) to be fully vaccinated (Covid 19) from the 11th November 2021, unless exempt under the regulations.
