Position Overview:
Greenlife Healthcare Staffing is seeking a dedicated and compassionate Registered Nurse to join a five-star rated Assisted Living Facility in Queens, NY. This role involves providing skilled nursing care, developing and implementing care plans, and ensuring compliance with regulatory standards in a supportive and collaborative environment.

Qualifications:
* Education: Associate/Bachelor’s Degree in Nursing.
* Licensure: Active New York State Registered Nurse license.
* Experience: Minimum of 6 months of clinical experience.
* Technical Skills: Proficiency in documentation and care planning.
* Soft Skills: Strong communication, organizational, and problem-solving skills. Ability to work independently and as part of a team.
Key Responsibilities:
* Performs a home care assessment and re-assessment of patient and environment to determine home health care needs. As indicated, uses the Oasis assessment tool.
* Calls the home care coordinator or supervisor with verbal report of initial findings.
* In conjunction with the family and physician and/or authorized practitioner and the facility supervisor develops and implements the Plan of Care based on the assessment.
* Provides skilled nursing care as outlined in the Plan of Treatment to include the following: Nursing services, treatments and preventive procedures including those requiring substantial specialized skill and ordered by the physician or authorized practitioner.
* Initiates preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety.
* Observes signs and symptoms and changes in the patient's physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary.
* Reports all patient changes and information to the supervisor or home care coordinator on a timely basis.
* Submits a record of visits/hours worked each day.
* Teaches, supervises, and counsels the patient and caregivers regarding the nursing care needs and other related problems of the patient at home.
* Documents services rendered, changes noted in the patient’s condition, and/or family and home situation, and evaluates patient response to treatment.
* Documentation of all care provided is in accordance with the facility's policies and procedures.
* Participates in Performance Improvement activities.
* Orients and supervises home health aide staff and documents supervision according to the facility's policies and procedures.
* Observe client rights and confidentiality.
* Observe infection control and standard precautions.
* Observes Advance Directives, as indicated.
