Job summary The post will be based within Ysbryd Y Coed, Older People's Mental Health Services. There are 3 wards within the unit, covering up to 60 beds. The post holder is identified as a clinical expert and is required to have a high clinical profile ensuring the provision of effective and efficient care. At this level the post holder is expected to work as a highly skilled senior practitioner and to have expert knowledge. The post holder will act as a practice leader, managing their own workload and work across professional, organisational, agency and system boundaries to improve services and develop practice. They will participate in networks, locally, including an already established ANP network within the Mental Health and Learning Disability Division, regionally and nationally and assess and manage risk and proactively challenge others about risk. Main duties of the job The primary focus of the role is to work as an advanced, autonomous practitioner providing patient-centred clinical care, encompassing the skills of assessment, examination, diagnosis and treatment of patients. The post holder will be working alongside and supervised by the substantive Specialty Doctor, specifically meeting and addressing the physical health needs of this patient population whist they are an in-patient having assessment and treatment for their Mental well-being. A key component of this role is admission avoidance to the acute general hospital setting as far as is possible for this client group. The post holder will be expected to work use their advanced clinical skills to undertake a range of physical examinations and investigations in the diagnosis and subsequent management of the patient need. This will also entail independent prescribing within scope. This is essential for this role. The post holder will be expected to work within their level of competence and to work flexibly as part of the team and will be expected to exercise a high degree of personal professional autonomy, make critical judgments to satisfy the expectations and demands of the role and will be guided by health, organisational or broad occupational policies, but in most situations the post holder will need to establish the way in which these should be interpreted and advise relevant stakeholders accordingly.
