Kingston Heart Failure Service has developed rapidly over the last 2 years to meet the ever-increasing needs of our ageing population. We are looking to recruit an innovative, passionate and dedicated Heart Failure Nurse Specialist, to continue to develop our excellent service. Working closely in conjunction with the Heart Failure Consultants, Consultant Cardiologists and GP partners, the post holder will provide a community heart failure service which will encompass home visits and community-based clinics. They will be an expert clinical leader, devising individualised programmes of care for our patients with heart failure and working closely with our GP partners to educate and support in the management of heart failure. This is an exciting opportunity You will work autonomously, whilst being an integral member of a wider team of Cardiac Nurse Specialists, currently leading services across the cardiac spectrum. Main duties of the job To provide high-quality, patient centred, community heart failure care. In conjunction with the KRFT Heart Failure Nursing Team and the Heart Failure Consultants, the post holder will work autonomously as an advanced practitioner managing patients in the community with heart failure. (This is a lone worker role, with remote support from the Cardiology team at Kingston and Richmond Hospital). The post holder will provide high level clinical assessment, care planning, expert advice, extended intervention and thorough evaluation. The post holder will work collaboratively with Primary and Secondary care teams to optimise the management of patients with heart failure, providing advice to primary and community colleagues, assist with the reduction of inappropriate referrals and unplanned admissions and ensure that patients with heart failure receive holistic and integrated care.
