We are a very friendly, rural practice currently operating from two sites in the villages of Burham and Eccles with plans to move to brand new purpose built premises within the Peters Village development sometime within the next 12 months. With our growing patient list from the residential development within our catchment area, we are looking for an enthusiastic individual to join our stable and cohesive practice team.

Responsibility, care and management of an allocated list of patients, undertaking all clinical and administrative tasks as needed ensuring the highest standards of care for all registered and temporary patients. To work at both sites of the practice as required.
About us
Additional clinical workload support from our GP principal, 2 salaried GP's; an Advanced Nurse Practitioner; a Prescribing Paramedic; 3 Practice Nurses; 2 HCAs; Medical PA and 2 administrators.
PCN workforce includes pharmacy team, First Contact Physiotherapists; Child Health & Wellbeing Navigator; Dementia Care Coordinator; Learning Disability Care coordinator; Social prescriber and paramedic led clinical assessment team.

Sessions & Days of Work: 5-6 sessions per week (open to applications from the GP Retainer Scheme)
Annual leave: Six weeks annual leave plus Bank Holidays (prorated)
Study leave: One week per annum (prorated)

Reports to: The Principal GP (Clinically) and The Practice Manager (Administratively)
Overall Purpose: To provide NHS personal medical services to registered patients on the Practice list.
Job Summary

Clinical Responsibilities
In accordance with the Practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties including surgery consultations, remote and telephone consultations and queries, triage, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. We currently have external provision for home visits via our Primary Care Network but this may change in the future.
* To provide duty doctor cover subject to when sessions worked.
* Respond to medical problems presented by temporary residents as required.
* Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
* Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
* Screening patients for disease risk factors and early signs of illness.
* In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.
* To assist in the establishment of appropriate systems to manage common chronic medical conditions e.g. Asthma, COPD, Diabetes etc.
* Completion of clinically related administrative and non-clinical duties needed for the delivery of the service.
* Providing counselling and health education.
* Refer patients for further opinions and interventions according to practice protocols.
* Recording clear and contemporaneous consultation notes to agreed standards using templates provided where appropriate.
* Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).
* Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
* Be prepared to take part in the evening or early morning rota for improved and extended access.
* Promote patient participation in clinical research trials that the Practice is engaged with, where appropriate.
* In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
Maintaining, Monitoring & Updating Computer Database and Management of Medical Records & Practice Audits
* Ensure that practice clinical database is updated promptly and accurately with information relating to patient records, as required for clinical accuracy and for practice recording requirements under IM&T protocols, QoF, QIPP, Enhanced Services and other clinical initiatives the practice partakes in.
* Respond to alerts on records.
* Appropriate use of read/SNOWMED codes (via templates where appropriate).
* Participating in the collection and collation of statistics towards the preparation of practice reports, performance monitoring and audits.
* Participation in the auditing of Practice activity.
* Participation in patient satisfaction audits.
* Participation in work involved for CQC compliance.
Practice Organisation
* Process and action incoming patient and hospital correspondence.
* Undertake private medicals as required.
* Attendance at primary health care team meetings, significant event audit meetings, annual complaint audit meeting and others as deemed necessary.
* Initiate and participate in the formulation and implementation of new protocols, policies and procedures to aid and improve administrative organisation and the provision of services to patients.
* Ensure that the Practice complaints system is adhered to at all times.
* Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety.
* Completion of Information Governance on line toolkit or attendance at group training annually and provide certificate.
* Maintain current CPR and anaphylaxis certification and safeguarding for adults and children certificates.

* Participate in an annual performance review under agreed headings.
* Engage in appropriate educational activity to maintain clinical competence and performance.
* Attendance and participation in educational meetings in the Practice and externally.
