Discharge Coordinator
Service care Solution are currently recruiting for a Discharge Coordinator in Northampton.
The Discharge Coordinator will be working in an integrated multi professional patient flow team and will be involved facilitating a safe and timely discharge for all patients on the ward.
Pay rate - £17.09 per hour / £21.14 ltd (umbrella)
Main Responsibilities
As a Discharge Coordinator, you will be responsible for:
1. Attending ward MDT meetings
2. Attending board rounds and co-ordinating all actions relating to the process
3. Completing discharge notifications for onward referrals to other discharge services
4. Completing all referrals to other services such as community nursing etc and checking that medication for discharge is available
5. Ensuring all transport arrangements are in place, arranging home assessment visits, progress meetings.
6. The post holder will be expected to communicate with patients who may have communication difficulties, families and other colleagues.
7. Support complex discharge home assessments if required
Requirements:
8. Foundation degree in health-related subject and/or relevant experience working at this level is essential.
9. Enhanced DBS
10. Experience working in a rehabilitation setting preferably at an inpatient unit is desirable.
