Job overview
An exciting and patient-focused role has become available to provide a high-quality Hospital Discharge service within Sheffield Teaching Hospitals NHS Foundation Trust to prevent hospital delays and re-admissions. The objective of the role is to provide a seamless discharge with all provisions in place for a safe discharge by coordinating all service provisions together that are required upon discharge.
The individual will be part of a team responsible for effective communication and coordination between wards, clinical teams, and services outside the hospital e.g. Adult Social Care.
The individual will need to proactively manage the Hospital Discharge waiting lists for all discharge pathways requiring service provisions.
The individual will also be part of the team working towards developing the Hospital Discharge HUB alongside Health & Social Care professionals.
Main duties of the job
1. To process and coordinate all referrals for Social Care and Community NHS Services from GP, Hospitals, and Health and Social Care professionals, in a timely and professional manner.
2. To receive and make telephone calls from/to other health and social care professionals.
3. Responsible for the coordination of hospital discharges for patients who require ongoing care to support them at home, including Health, Social Care teams, and Independent Sector provider services.

Qualifications
* Educated to GCSE standard; at least five GCSEs Grade C/4 and above, including Maths and English or equivalent qualification/experience.
* NVQ Level 2/3 in Business Administration or equivalent relevant experience.
* ECDL or equivalent proven skills to the same standard.
* Evidence of continuous professional development.
* Any further educational qualifications are a distinct advantage.
Experience
* Experience of working in a multidisciplinary patient-centred environment.
* Experience in dealing appropriately with highly sensitive and confidential information, demonstrating a keen eye for detail.
* Experience of working in a busy office and highly pressurised environment whilst working to tight and changing deadlines with a number of conflicting demands.
* Ability to use own initiative to problem solve complex situations that arise.
* Proficient in the use of IT packages, particularly Microsoft Office.
* Demonstrated effective communication skills and ability to deal effectively with conflict to minimise its escalation with all levels of the organisation.
* Experience of collating, recording, and maintaining databases to enable accurate data reporting.
* Experience of dealing with a high volume of telephone enquiries.
* Extensive and relevant experience in Health and Social Care.
* Knowledge of medical terminology.
* Knowledge of relevant Trust policies and ability to ensure compliance.
* Ability to identify Safeguarding Concerns and follow the escalation process.
* Ability to demonstrate negotiation and tactful skills to ensure patient needs on discharge are met.
* Ability to multitask and prioritise a diverse workload, meeting deadlines due to the high demands of the service.
* Ability to show accuracy and attention to detail.
* Skills and initiative to problem solve complex issues that will arise both internally and externally.
* Confident in dealing with colleagues, patients, and other organisations in a calm, patient, and tactful manner, including in difficult situations.
* Ability to work on own initiative, taking ownership of your allocated workload.
* Willingness to undertake further training and development.
* Ability to identify areas for improvement and propose achievable solutions.
* Working knowledge of Trust Systems e.g. SystmOne /Lorenzo, Insight, Patient Discharge Planning System, and Sheffield City Council Liquid Logic and Azure.
* Ability to interpret information requirements and present complex findings.

* Excellent knowledge of the NHS Trust & Social Care.
