Main duties of the job
* To assess, plan and coordinate safe, timely discharges via the most appropriate discharge pathway to meet the patients' required needs.
* Manage a caseload of patients with varying diagnosis and/or complex needs with a range of disabilities including physical, learning and mental health disabilities.
* To educate and train the team on the role of OT and provide day-to-day management for junior staff, ensuring supervision and annual review.
* To participate in the planning, development and evaluation of the discharge team service within the acute trust and the wider integrated services.
* Take a lead for information management to ensure high quality data which is recorded, evaluated & reported in a timely manner.
