To ensure communication is clear and carried out in a way that consistently displays professionalism at all times to ensure effective working relationships with colleagues, clients/patients and other professionals/agencies. The post holder will communicate in person, via telephone calls, emails and MS Teams and ensure information is provided and received in line with GDPR and Information Governance. This will require high levels of interpersonal skills and a level of tact and empathy. This will also require advising and signposting to appropriate services. Provide a presence and support to designated wards/departments to facilitate safe and timely discharges from hospital including attendance at board rounds to partake in setting PDD and identify and resolve any barriers to achieving PDD. To report daily into the IDH management of updates on patients discharges. Attend meetings/huddles for allocation of patients to commence discharge planning. Provide Information and support to patients, families and staff in relation to complex patients discharge from initial contact to 72 hours post discharge Lead and/or participate in MDT meetings, discharge planning/professional and best interest meetings to support patient discharge Work alongside the other members of the IDH to promote the IDHs role in discharge planning. This involves educating and empowering other professionals to be involved in discharge planning. Work with the MDT to implement discharge requirements for each patient on admission to the ward and ensure early referrals. Recording all information regarding the patient in the electronic patients records. To ensure that there is discharge practitioner representation during the operational hours of the IDH To manage complex and sensitive information on a daily basis, using empathy and reassurance skills at all times with all people. Ensure that information is recorded and shared in line with information governance. To contribute to the rota management. Ensure relevant databases used in discharge planning are updated to enable accurate oversight of patients length of stay and any barriers to discharge. To contribute in any system events related to patients discharge and length of stay (e.g. escalation). Ensure that information relating to a patients discharge is accurately recorded and kept secure on the appropriate database/system and the electronic care record. Ensure that the referral to IDH is completed in full as soon as possible after admission, for patients needing or requiring support with discharge by the appropriate professional. To collaboratively work with the patient and/or representative along with MDT professionals to set goals for discharge and beyond. Ensure that at board rounds the MDT sets the PDD as per national golden standard (within 48 hours of admission) and identify and resolve any barriers to achieving PDD To lead on complex discharge planning working with the MDT to ensure discharge is safe and seamless. To resolve barriers in relation to a complex discharge and escalate further as and when necessary in order to facilitate safe and efficient discharge. To work collaboratively with and delegate or share duties as appropriate to other members of the IDH to coordinate complex discharges.
