Key Responsibilities: - Advise on the Trust strategy for new models of care in conjunction with the General Manager, Clinical Director, Associate Chief Nursing Officer and Associate Chief Operating Officer for the Emergency Division and other Divisions as appropriate. Clinical responsibility for acute pathways within a defined specialty. The provision of expert clinical practice in regard undertaking highly specialist roles across the specialty, ensuring patients are enabled to be discharged earlier from hospital or preventing admission. This will include providing clinical leadership in clinical practice; providing professional and clinical leadership to the Advanced Clinical Practitioners, Professional leads and specialty nursing teams where appropriate; leading and participating in advanced Nursing/Therapist practice. The post holder will work in clinical practice for 50% of time. Responsible for the interpretation of National policy in the development of services and Trust related outcome measures, policies, guidelines, protocols and procedures. To contribute to the development of new models of care to transform the care provision within the Trust ensuring sound and effective clinical pathways are developed, implemented and evaluated. To undertake research and audit activity in evaluating models of care and informing national policy. To provide an expert consultancy service to patients, carers and colleagues, both within the Trust and across wider networks. To provide professional leadership in a way that inspires colleagues, improves clinical standards and quality and develops professional practice. To provide advice and leadership in the education, training and development for the service delivery across the acute, community and primary care sectors.
