Responsible for the setting up and continued development of the nurse led arrhythmia service, working closely with the Heart Rhythm Specialist and Cardiac Physiologists to provide a comprehensive service to patients presenting with complex cardiac conditions. Responsible for the day to day organisation of the nurse led specialist service for patients with abnormal heart rhythms. Set up and run nurse led clinics for patients referred into the service. Out patient clinics will be held in both secondary and primary care settings, working closely with cardiologists, GPs and cardiac physiologists. As part of the Community Cardiology Project, the post holder will see patients referred by their GPs for investigation of palpitations in a primary care clinic, e.g. Keir Hardie Health Park. Undertake a comprehensive assessment of each patient, including history taking and physical examination. Refer patients for appropriate investigations as required to assist in the accurate assessment and diagnosis of individual patients, e.g. echocardiogram, 24hr tape recorders and event recorders. Provide specialist education and counselling to patients and their families. Liaise with others in the multidisciplinary team, e.g. Pharmacists, Anticoagulation Service, and Palliative Care Team including Macmillan Service. Take referrals for patients with new onset atrial fibrillation and assess their individual needs in a timely manner. Working closely with the Consultant Cardiologist, the nurse will be responsible for providing education, support, optimising medication including anticoagulation. The nurse will be required to arrange further investigations and referral to the Cardioversion Service. Be responsible for the development and implementation of evidence based protocols and guidelines for the nursing service. Work closely with the multidisciplinary team when reviewing or developing service policies.
