Post: Higher Needs Floating Support; 1x High Support Housing Cluster Worker and 2x Out of Hospital Settle-In Workers
The Higher Needs Floating Support Service is a newly commissioned service working with people in temporary accommodation or at risk of homelessness across a variety of settings. The team works closely with the local authority, health, treatment, and specialist support providers to improve outcomes and work towards more settled accommodation for those facing multiple disadvantages.
As a Floating Support Worker within the Higher Needs Floating Support Service, you will:
* Work to make current accommodation sustainable, identify new or more appropriate accommodation with the local authority, and work in a motivational and psychologically informed way.
* Engage and support clients with complex needs and/or facing multiple disadvantages to maximize their choice and control over the support they receive by developing a network of support.
* Collaborate with landlords, support agencies, hospitals, healthcare, family, friends, and others to create a comprehensive network of support that remains beyond the end of support provisions.
You will:
* Have experience supporting people facing multiple disadvantages.
* Have some experience improving housing outcomes for people who have been homeless.
* Be great at building trust with those you work with.
* Have compassion, empathy, and passion for supporting others.
* Be IT literate and able to record and report information.
* Have confidence in lone working in the community.
* Have the ability to travel across Bristol.
