About the role Your first week includes a day 1 induction and equipment collection, followed by 4 days of training from home fully remotely. As a Registered Nurse with CHC experience, you will chair Multi-Disciplinary Meetings (MDTs) and conduct reviews and assessments on behalf of Integrated Care Boards (ICBs) and Health Boards. These assessments will be carried out virtually via MS Teams or Zoom, involving the individual, their provider, and family (or their representatives). Communicate objectively complex condition-related information to individuals and/or relatives professionally whilst demonstrating empathy and compassion. Effective negotiation, persuasion, and interpersonal skills. Managing multidisciplinary stakeholders effectively through face to face, written and telephone meeting, leveraging on your experience to deliver value to ICB. Handle sensitive and complex information about service changes professionally. Create clear reports accessible to both individuals and clinical audiences. Assess and interpret specialist conditions with analytical and judgement skills. Plan and organise effectively to achieve group and personal targets. Knowledge of care plan provision processes in the UK for individuals. Role requirements NMC Registered Nurse. Minimum 12 months post-qualification experience. Previous experience working within an NHS Continuing Health Team or in the community completing CHC assessments and reviews. Experience of using the CHC Decision Support Tool, case review, retrospective review. Experience of chairing MDT meetings. Ability to write clear and comprehensive reviews and assessments to a high standard. Able to complete person-centred care and support plans. Computer literate, specifically for Microsoft suite including Outlook & Excel. Experience in similar work; reviewing, evaluating, and planning adult health and social care.
