Your role and responsibilities This Senior Practitioner role offers an exciting and unique opportunity to support the overall management of the team, with leading responsibilities on the safe and effective review and discharge co-ordination of adults with S117 aftercare rights. You will continue the integrated partnership working with NSFT and health professionals to ensure timely reviews and support joint funding discussions with the Integrated Care Board. You will work with the leadership team to manage demand whilst delivering the highest standards of social care. Working closely with the team supporting day to day practice and ensuring quality service provision. You will provide professional support to practitioners encouraging growth and development of skills and knowledge of the team modelling leadership. You will also be part of the Ipswich and East Leadership team Working alongside Operations and Partnerships Managers and the wider Senior Management Team and health partners you will work to deliver the council’s political and strategic agendas. You will: support aftercare services for those who have experienced mental health detention sollaborate with various health and community services to improve outcomes for people with mental disorders, including dementia promote best practices in social work to enhance clients' lives manage complex cases, supporting discharge from acute later life mental health ward participate in ward discussions and care planning activities attend senior meetings to maintain patient flow and escalate concerns supervise practitioners, including AYSE or Apprentice Social Workers You will need a social work qualification Social Work England Registration. The team The Ipswich and East Optimising Care and Support team is based primarily in Landmark House but works geographically across the eight Integrated Neighbourhood Teams. There are two key elements to the team; one is to support the ongoing management of S117 aftercare cases for those with organic mental health issues and some legacy cases with functional mental health needs. This includes supporting discharge pathways from later life acute mental health wards. The other is to proactively respond to targeted reviews. Developing person centred planning and seeking creative solutions to meet needs that optimise independence and service provision within Ipswich and East Locality.
