Job summary Responsive services have opening for a band 6 community sister/ charge nurse in their virtual ward.This is an exciting opportunity to join a friendly and vibrant team working under the supervision of the virtual ward matron. Virtual wards allow patients to receive the care they need at home safely and conveniently rather than being in hospital. Enabling acutely unwell patients to be cared for in their own home, with support of technology. The service works 7 days a week from 8am 8pm. You will work with the multi- disciplinary team to provide outstanding care to your patients and their families. Main duties of the job The postholder will work in a caring, supportive, and professional environment, providing person centred nursing care to individuals on the virtual ward, promoting their health and independence. The community sister/ charge nurse will need to be able to work autonomously and as part of a team, supporting junior clinicians to develop skills in delivering care to patients. The postholder will undertake a thorough holistic assessment, with physical assessment skills being desirable or a willingness to undertake additional training to gain these essential skills. The community sister/charge nurse will be supported to advance their critical thinking and decision-making by the virtual ward matron and consultant lead frailty team to deliver safe competent nursing care.
