Job summary Job Summary We are seeking a Primary Care Advanced Clinical Practitioner (ACP) / Trainee Advanced Clinical Practitioner (tACP) to join our dynamic team. This role is suitable for both experienced ACPs and those who have recently qualified. The successful candidate will provide high-quality, patient-centred care, working autonomously within their scope of practice to assess, diagnose, treat, and manage a wide range of conditions. For ACPs, we welcome applications from both experienced practitioners and those newly qualified, with support available to help develop confidence and competence in the role. For tACPs, a structured training pathway will be provided to support progression towards full ACP status.
