Main Duties of the Job
Eleanor Frailty unit is a 12 bedded unit and mainly deals with frail patients who need less than 72 hours period to be discharged. We are a progressive ward, with a focus on staff development and advancement with the aim of delivering the best possible care to our patients.

Job Description
Job Responsibilities
Eleanor frailty unit works closely with Frailty practitioners and Accident & Emergency department.
We prioritize staff development and progression to ensure the best possible care for our patients. Support, specialized knowledge, and expertise will be needed for the daily care and management of all patients in the ward.
You will have the chance to showcase your clinical, managerial, and leadership skills in this fast-paced environment.
The Post Holder Will Be Responsible For:
1. Deliver high quality nursing care tailored to frail, elderly patients with multiple comorbidities.
2. Assist in medication administration and review, ensuring appropriate prescribing.
3. Implement individualized mobility and rehabilitation plans with physiotherapists and occupational therapists.
4. Identify and mitigate the risks related to falls, infection, and pressure injuries.
5. Maintain a high standard of patient-focused care.
6. Maintain a safe and clean working environment, providing assessment, planning, implementation and evaluation of nursing care for patients ensuring accurate written records are kept.
7. Communicate with the patients and carers effectively to ensure they have access to appropriate information.
Requirements
1. Provide advice and education to patients, staff and learners within the ward as required.
2. Develop communication networks across specialties and throughout the multidisciplinary team.
3. Prioritize their own workload and that of those working with them such as learners and Healthcare Assistants.
4. Carry out nursing care to the highest possible standard according to Trust Policies and Practice and Clinical Guidelines.
5. Know what Essence of Care is and how it affects patient care and needs and escalate concerns.
6. Know how to deal with people with challenging behaviour and to know when to call for assistance when needed.
7. Adhere to uniform policy.
8. Liaise with the bed manager and discharge co-ordinator to ensure effective bed management on the ward.
9. Ensure that all equipment is maintained, and any faults immediately reported.

Education, Training & Qualifications
Essential
* Completion of a nursing degree programme.
* NMC Registered Nurse.
Desirable
* Mentorship or teaching qualification.
Knowledge & Experience
Essential
* Previous experience in managing elderly patients.
* Recent experience working in any geriatric wards or looking after Frailty patients.
* Evidence-based practice relating to a ward nursing environment.
Desirable
* Knowledge and understanding of the trust's nursing strategy.
Skills

* Demonstration of team working and ability to work effectively within a team.
* Demonstrate an ability to prioritise own workload and recognise, prioritise and respond appropriately to urgent and emergency situations.
