Leigh PCN (Leigh GP Practices) Salary: Grade 6, 28,624 - 31,586 per annum. 37 hours per week Your role: Within your role you will help to transform the lives of individuals and their families/carers by enabling people to get access to the right support at the right time and to feel active and confident in managing their own health and wellbeing. You will act as the single point of contact for individuals to ensure appropriate support is made available to them and their families/carers, supporting them to understand and manage their condition and ensuring their changing needs are addressed and enabled. About you: The ideal person for the role as a Care Coordinator will have a range of skills and life experiences. You will be organised, with time management skills and the ability to work flexibly within and team and as an independent worker. The most important attributes for this role will be your personal skills, with the ability to demonstrate care and compassion.
