Main duties of the job Assess, plan, implement, and evaluate care for alcohol dependent patients with complex needs, making necessary adjustments. Identify patients at high risk from their alcohol use using the AUDIT assessment and take appropriate action in collaboration with the interdisciplinary team. Deliver evidence-based interventions, including motivational interviewing and withdrawal management, to support patients in reducing alcohol-related harm. Educate patients and families about healthy lifestyles and managing risks to health and well-being. Ensure accurate, up-to-date documentation using electronic health records and maintain compliance with the NMC Code of Professional Conduct. Communicate effectively with patients, staff, and external organizations, using strategies to overcome barriers and promote continuity of care. Support research, audits, and quality initiatives to enhance patient care and nursing practice. Oversee the delivery of a 7-day alcohol care service model to ensure consistent and equitable coverage for patients across all settings.
