The successful post holder will provide a specialist resource as a Paramedic/Paramedic Practitioner for our patients working with clinicians, nursing team and in collaboration with other members of the practice multidisciplinary teams. You will work within our practice community, as an autonomous, accountable paramedic practitioner providing holistic care whilst assessing, diagnosing and providing treatment to patients. You will also assess, diagnose, treat, refer or direct to other services; our patients who attend surgery or request domicile visits relating to unscheduled minor conditions or acute illness. You will promote self-care and educate service users to allow them to be empowered to make informed choices about treatment. Main duties, tasks & skills required Shakespeare Road Medical Practice have around 14,000 patients and this exciting role will be pivotal in supporting meaningful change and alignment of services within the Practice and PCN. The Paramedic role will provide a fantastic opportunity for a driven and passionate individual to drive innovation and support the ongoing excellent care of patients in the Basingstoke area. The Role will be supported by a GP prescribing Lead. The post holder is an experienced paramedic who aspires to provide a responsive, safe, effective, and caring service to patients. Role involves initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinic decision making and expert care, including assessment and diagnostic skills, for patients within the general practice setting. Along with using a medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team. The clinician will participate in, and influence efforts, across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or as close to the patients' own home as possible.
