Job summary Job Summary The post holder is an experienced Clinical Pharmacist, who acts within their professional boundaries, to develop, manage and mentor a team of pharmacists. The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take a leading role within the practices for areas of chronic disease management and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities. The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support a management system for medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practices. The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. Main duties of the job Clinics To see patients independently - making decisions about their care and treatment as appropriate - GPs will be able to refer patients into this clinic. These include; Complex polypharmacy patients with a view to review all their listed medications, make plans for ongoing treatments, de-prescribe where needed etc. Patients from a particular cohort e.g. on high risk drugs. The role here could be to counsel, check for side effects, review dose/need and check indication is in line with national standards. I will make changes to treatment where appropriate. To assess and triage patients, including same day triage as needed, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team; To manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action; To use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice; Lead Pharmacist Role Management of PCN Pharmacy Team Senior pharmacist within PCN pharmacy team (as per PCN DES).
