What you will do:
1. Provide advanced assessment and treatment for patients with complex needs, ensuring high-quality care.
2. Offer expert advice, guidance, and information to patients, carers, relatives, and healthcare professionals.
3. Manage the early assessment of patients referred from A&E, Same Day Emergency Care (SDEC), Enhanced Care Unit (ECU), and Acute Medical Unit (AMU).
4. Collaborate closely with community services and partner agencies to maintain seamless continuity of service.
5. Provide insightful expert advice within the multidisciplinary team.
6. Supervise and support students, Band 5 staff, and assistant staff members, fostering a learning environment.
Requirements:
1. A degree or Graduate Diploma in Physiotherapy is essential.
2. Evidence of continuing professional development.
3. Knowledge of relevant National Service Frameworks (NSFs) and national guidance.
4. Proven ability in complex case management.
5. Excellent communication and core IT skills.
6. Evidence of supervisory experience.
