Main duties of the job What You'll Do: - Ensure smooth and timely processing of patient referrals using the PRIS Referral system. - Track and monitor patient progress through specialized care pathways, supporting MDT team meetings. - Collaborate with medical professionals across disciplines and communicate with external agencies to ensure seamless care delivery.
