The Digital Care Co-Ordinators are expected to work as a team. They will be assigned to 1 or 2 specific areas in SWL (Kingston, Richmond, Croydon, Sutton, Merton and Wandsworth). The digital care co-ordinator is a flexible and proactive team member with the capacity to support the Primary Care Networks and practice teams to deliver patient facing support to their patients, in the transition to the digital era. As a digital care co-ordinator, you will be expected to support patients registered at SWL GP Practices, to use digital technology. This role requires strong interpersonal skills as well as coaching, and mentoring skills. The post holder will be expected to establish and maintain key local relationships with practices, PCNS and patient groups within the SWL footprint, which actively contributes towards successful uptake of modern general practice tools among the population. Digital care co-ordinators must stay informed on the developments in primary care and devise ways for them to be promoted and integrated with SWL patients. Key Working Relationships The postholders will be required to maintain good working relationships with a broad range of internal and external stakeholders, including: SWL locality Training Hubs SWL Primary Care Transformation team SWL Change Managers Clinical Leads Primary Care Networks and member practices SWL patient groups including Healthwatch Community Groups Community pharmacy Corporate Responsibilities Support the SWL ICS Training Hub and SWL ICB Primary Care Transformation Team to deliver high quality projects. This will be achieved through effectively fulfilling the main duties of the role detailed above. Demonstrate sound judgement in the absence of clear guidelines or precedent, seeking advice as necessary from peers or the managerial and clinical lead as appropriate on any projects or work within the postholders remit. Continuous Professional Development The post holders will pursue a programme of CPD in accordance with any relevant professional registration or statutory requirements, whilst maintaining appropriate awareness of service provider requirements. Risk Management Responsibility for ensuring that risks are managed at work group and local level. This process will include the assessment and effective control of all acceptable risk situations.
