Clinical Lead Dietician Community Integrated Care Band 7
The Medical People are recruiting for a hospital in Carmarthen, Wales.
Job Role: Provide a specialised clinical dietetic service for the acute/urgent response team and surrounding integrated community teams by leading on training, education, and development of support workers and the wider community teams. This role aims to embed current policy and best practice to take early action on malnutrition, reduce the impact of delayed response times, and increase opportunities to identify and manage malnutrition risk proactively through effective and sustainable service delivery.
Main Duties and Responsibilities:
1. Manage a clinical caseload of adults with varied and complex nutritional support needs as an integral member of the community integrated teams.
2. Support the timely identification of patients at risk of malnutrition and proactively upskill others across the wider multi-professional team.
3. Act as dietetic lead and key member of the community team, collaborating with team members including nurses, therapists, pharmacists, and support workers.
4. Provide clinical advice to the rapid response team/community resource team and other professional agencies.
5. Use specialist adult dietetic knowledge and skills to assess, plan, implement treatment, and deliver dietetic interventions in various settings.
6. Contribute to the development and implementation of policy, guidance, and standards relevant to nutrition and dietetics.
7. Demonstrate autonomous clinical judgment when assessing complex patient presentations within the field of Dietetics.
8. Evaluate dietetic and multi-disciplinary interventions and outcomes to ensure effective and patient-focused dietetic services.
9. Participate in audit/research within the service and multi-disciplinary teams.
10. Ensure equality and diversity issues are recognized and addressed in accordance with legislation, policies, and procedures.
Maintains a clear understanding of the philosophy and theories underpinning dietetics and the role and responsibilities of the post.
