Wentworth 1 Primary Care Network is made up of several GP practices in the Rotherham area. We are recruiting a number of Advanced Clinical Practitioners to work in our friendly GP Practices.
We are recruiting for an Advanced Clinical Practitioner to work across Magna Group Practice which has 4 sites. You may be asked to work across more than one site.
The Advanced Clinical Practitioner will utilise a range of advancedskills and expert knowledge for patients presenting with undifferentiated andundiagnosed conditions. The post holder will demonstrate critical thinking inthe clinical decision-making process. Clinical care will include new patientclerking, initial history taking, face-to-face assessment, examination,diagnosis, treatment, management, referral, and evaluation of care. You must possess a Digital Badge as this role is funded via the ARRS scheme.
Main duties of the job
* Work autonomously and be accountable for his/her own professionalactions.
* Undertake clinical decisions at an advanced level using expert knowledgeand clinical skills to deliver holistic care to people accessing primary careservice.
* Assess undifferentiated patients with a range of acute, non-urgent andchronic medical decisions.
* Obtain a clinical history, undertake appropriate physical examinations,and investigations, formulate differential diagnosis and carry throughmanagement treatment plans ensuring relevant follow up.
* Undertake pre booked emergency clinics, face to face consultations,telephone or video consultation.
* Working as part of a larger primary care network to improve patient careand ensure more efficient ways of working.
* Promote a learning environment for patients, nurses and other healthprofessionals.

This includes the creation of additional roles in our practices including Clinical Pharmacists, Social Prescribers and Care Coordinators.
Job responsibilities
* Maintain their professionalregistration. Work within the most up to date relevant professional standardsset by individual profession governing body.
* Record accurate consultation data inpatients records in the clinical systems (eg: EMIS, System One) in accordance with theup-to-date professional standards
* Make effective use of the clinicalsystem by ensuring consistent and accurate data entry and use of read codes.
* Instigate necessary invasive andnon-invasive diagnostic tests or investigations and interpret findings/reportsto discuss with clinician as appropriate.
* As an independent prescriber, thepost holder will prescribe safe, effective and appropriate medication asdefined by current legislative framework within scope of practice.
* Provide safe, evidence-based,cost-effective, individualised patient care
* Offer a holistic service to patientsand their families, developing where appropriate an on-going plan ofcare/support, with an emphasis on prevention and self-care and support patientswithSocio-economicissues.
* Promote health and well-beingthrough, health education, screening and therapeutic communication skills toimprove self-care and promote independence for patients.
* Participate in screening patients fordisease risk factors and early signs of disease.
* Refer patients directly to otherservices/agencies as appropriate or after discussion with a GP.
* Undertake home visits, (includingcare homes), telephone/video consultations, telephone triage and support futuredevelopments in order to improve service delivery and patient experience.Encouraging patients to rely on face-to-face consultations only where there isclinical need to do so.
* Triaging Patients for GP emergencysurgery and support the on-call GP as appropriate.
* Working with allied healthprofessional to ensure Quality and Outcome Frameworks are delivered
* Under complex decision making whichinvolves sensitive or contentious information and occasionally deal withsituations which are hostile or emotive.
* Exercise professional judgment tomanage risk appropriately especially where there maybe complex and unpredictableevents and supporting teams to do likewise to ensure safety of the individualsfamilies and carers.
* Use enhanced communication,negotiation, and conflict management skills and demonstrate empathy,reassurance and adopt persuasive skills to overcome barriers to acceptance.
* Work with multi-disciplinary teamwithin the Practice, and across the wider primary care network to promoteintegrated and seamless pathways of care
* Monitor the effectiveness of theirown clinical practice through the quality assurance strategies such as the useof audit and peer review
Person Specification
Qualifications
* a. has a masters degree level in the relevant area of expertise;
* b. is working at a masters level aware or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies;
* have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice.
* Must be able to prescribe.
Experience
* Evidence of both team and autonomous working.
* Can show evidence of using autonomous physical examination and assessment skills.
* Can demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children.
* Consolidated Independent Prescribing experience.
* Use of examination skills in practice.
* Experience of telephone triage.
* Experience of working in Primary Care.
* Experience of working in a GP Practice.
