About the Role
Deben Health Group PCN is looking to welcome a qualified and experienced clinical pharmacist into our busy practices, to play a full and active role within our multi-disciplinary team. The role will be based from Riduna Park, Melton and Woodbridge but you will be required to travel across all surgeries within the PCN. This position is offered on a full or part-time basis depending on the individual's circumstances.
The purpose of this important role within our PCN, is to lead improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care. The post holder will clinically assess and treat patients, using expert knowledge of medicines, for specific and complex disease areas.
Main Duties
1. Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
2. Providing pharmaceutical medication review & health education advice to patients, face-to-face, via telephone or notes-based reviews.
3. Carrying out structured medication reviews in care homes.
4. Undertaking clinical medication reviews to proactively manage people with complex polypharmacy.
5. Taking a central role in the clinical aspects of shared care protocols, clinical research with medicines.
6. Carrying out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), actioning, clinical letters and discharge summaries, and drug monitoring.
7. Supporting initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.
8. Reviewing daily pathology results.
About the Candidate
You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:
Essential:
* Registered with General Pharmaceutical Council (GPhC).
* Membership of the Royal Pharmaceutical Society.
* Accredited Master degree of pharmacy (MPharm) or equivalent.
* Independent prescriber or working towards/intent of gaining an independent prescribing qualification.
* Completed, or working towards, a Primary Care Pharmacy Education pathway qualification (CPPE).
* Minimum of 2 years (or equivalent) post qualification experience.
