Weare looking to appoint a Physiotherapist to join the Intermediate Care teamwhich is part of the Neighbourhood Rehabilitation Service (NRS) working acrossBarnsley Community.
The teamprovides rehabilitation to patients in their own home & residential care.The service provides specialist community therapy assessment & treatment toimprove health & independence. In addition, the service offersrehabilitation to enable earlier discharge from hospital or to prevent hospitaladmission.
You will assess, manage & provide treatment to individualswith complex and/or multiple needs, working closely with the MDT providingsupervision/support to other staff

A vehicle to undertake travel is essential &the ability to work flexibly including evenings, weekends & bank holidays

Main duties of the job
As Physiotherapist you will:
Manage a clinical caseload with autonomy.
Demonstrate knowledge, experience & relevant clinical skillsfor patients with complex/long term conditions, falls, orthopaedics &general mobility problems.
Undertake comprehensive assessment, provide therapeutic diagnosis usinginvestigative, advanced analytical skills, specialist listening, physical &observational skills, interpret clinical findings/documentation & implementagreed treatment plans.
Monitor & evaluate treatment to measure progress/outcomes to ensureeffectiveness of interventions & service delivery.
Develop programmes of care to support discharge.
Contribute to clinical leadership/supervision facilitating aculture of learning/reflective practice.
Havehigh level communication & excellent interpersonal skills.
Compliance with clinical governance being responsiblefor ensuring Infection Control policy / procedures are followed.

To work as an integral member of thePhysiotherapy and Clinical Teams within the service.
To carry out a specialised clinical caseloadundertaking specialist assessment, treatment and advice to patients often withcomplex presentations and formulate a clinical diagnosis and physiotherapytreatment plan as an autonomous practitioner.
To be responsible for specialist advice tostaff, patients and carers.

3.Core Duties and Responsibilities
CLINICAL
To be professionally and legally responsiblefor all aspects of own work including the management of patients in your care.
To perform specialist physiotherapeuticassessment and treatment of patients with diverse presentations, multiplepathologies and complex condition, formulating a diagnosis and treatment planusing specialised clinical reasoning skills and manual assessment techniques,within the clients own home, residential or nursing home.
To display specialist problem solving skills torecognise and identify patients requiring further investigations, referringappropriately to other services within and outside the physiotherapyremit. To be aware of and screen for seriouspathologies with appropriate management.
To hold responsibility for own caseload,working without direct supervision.Access to advice and support is available from more senior colleagues asrequired. Clinical work is not routinelyevaluated.
To formulate and deliver individualphysiotherapy treatment programmes based on a sound knowledge of evidence basedpractice and treatment options patient education, exercise programmes,electrotherapy and other alternative options.
To interpret and analyse clinical and nonclinical information to form a diagnosis, prognosis and to recommend the bestcourse of intervention, developing comprehensive discharge plans.
To assess patients understanding of treatmentproposals, gain valid informed consent and have the capacity to within a legalframe with patients who lack capacity to consent.
To communicate effectively with patient andcarers, overcoming barriers to communication and understanding in order toprogress rehabilitation and treatment programmes.
To provide planned and spontaneous advice,teaching and instruction of patients, relatives, carers and other health careprofessionals to promote the understanding of rehabilitation goals and ensure aconsistent approach to patient care.
Where necessary to be involved in MDT meetingsto ensure the delivery of a co-ordinated service to patients.
To work as a lone practitioner when working ina community or clinic setting with telephone support if required.
To be responsible for risk assessment of owncaseload and working environment and ensure the correct and safe use of allequipment and appliances by patients, and by staff and students throughtraining and supervision of practice.
PROFESSIONAL
To be responsible for maintaining owncompetency to practice through a variety of CPD activities and maintain aportfolio which reflects personal development.
To maintain and develop up to date specialistknowledge of Evidence Based Practice within a variety of pathologiesexperienced in area of work.
Undertake measurement and evaluation ofspecialist treatment and current practice through use of evidence basedpractice and outcome measures. Recommendchange of practice to service manager and to be involved in the implementationof changes to practice and service protocols.
Maintain HCPC registration, Trust, CSP anddepartmental standards, guidelines and policies ensuring quality of service andmonitor own Band 5 and assistants quality of practice.
To induct and provide clinical education,support and training to Band 5 physiotherapist and assistants assessing andevaluating competence as required.
To participate when required in the clinicaleducation of medical professionals in your specialist field of physiotherapy.
To partake in the clinical supervision andeducation of physiotherapy students, whilst maintaining communication linkswith the universities.
To participate in the Trusts appraisal system.
Be an active member of the in-service trainingprogramme by attendance at and delivery of training sessions, peer review andexternal courses where appropriate.
To ensure accurate, legible and comprehensivetreatment records are kept in line with CSP and trust standards of practice.
Establish and maintain communication links withother medical specialities in representation of physiotherapy services, toensure the delivery of a co-ordinated multi-disciplinary approach.
To accept responsibility from and delegate toother members of the physiotherapy staff.
To be aware of health and safety aspects ofyour work and implement any policies deemed necessary to improve safety.
