Job overview
Our Mental Health Outreach service assists clients who live in a supported living setting and independent living in the community. We require caring individuals providing emotional support and helping clients who self harm.
As a Support Worker, you will provide high quality care and support to our clients to enable them to live their best life. You will be passionate in making a difference to our client’s lives, helping them to fulfil their aspirations and ambitions.
Main duties of the job
* You will have a caring, confident and well organised personality with the ability to adapt to situations quickly as every day is different.
* It is desirable (not essential) that you have some experience of working in a care environment.
* Proven track record of teamwork and working in a fast-paced environment.
* You must be flexible as you would be required to work a variety of shifts, and also cover at other local services in the absence of other staff.
* You must be confident in speaking to other professionals and services who work with our clients, as you will be in regular communication with them.
* Incidents may occur, so you must be able to manage these calmly and report any safeguarding concerns to the local authority/any relevant professionals who are supporting the client.

Detailed job description and main responsibilities
* Mental Health support worker to our Outreach clients who live in a supported living setting and independent living in the community.
* You will be responsible for providing day to day support to our clients, including providing emotional support and helping clients who self harm.
* Our clients will look to you to help them develop their coping skills and how to manage their mental health safely.
* You will support and encourage our clients with money management, attending appointments, support with their medication and maintaining positive relationships.
* As a Support Worker in Outreach, you will also support our clients in exploring new interests, cooking/cleaning and improving on their physical health. This includes supporting clients in social/leisure activities within both the home and the community.
* You will have a person-centred approach and will promote culture and choices.
* As well as physically and emotionally supporting our clients, you will be responsible for maintaining accurate records and will write clear and concise daily notes.
