The East Midlands Planned Care Centre (EMPCC) is a vibrant new Elective Care Hub opening December 2024. EMPCC will provide a multi-professional approach, delivering the right care in the right place by a skilled and expert team. Full or part-time hours are considered, requiring work until 6 PM and occasional Saturdays.

Main Duties of the Job
The post holder, in conjunction with their team, will be responsible for the administration and coordination of a patient's pathway, from receipt of referral through to outpatient clinics.
Job Responsibilities
The post holder will be responsible for ensuring all relevant paperwork, medical notes, and letters are available for patient appointments.
* Prepare patient lists from the relevant information system in advance of the clinic, ensuring that all additional documentation is requested with sufficient notice.
* Adhere to the appropriate guidelines for the overbooking of clinics.
* Search for and request all missing notes in line with CMG guidelines.
* Liaise with Patient Pathway Coordinators, Medical Secretaries, and clinical staff regarding changes to appointments.
* Deal with queries and enquiries regarding appointments.
* Provide a reception service to patients attending appointments.
* Check patient demographic information at appointments and update records accordingly.
* Monitor appointment waiting times and inform patients of any delays.
* Ensure HRG sheets are completed with correct procedure codes.
* Maintain the accuracy and integrity of data recorded on paper and electronic systems.
* Scan and upload documents produced during appointments to electronic systems.
* Input/transfer case note data onto the relevant IT system.
* Update the relevant IT system with any suspensions, cancellations, and DNAs.
* Ensure investigation results are signed by medical staff before filing.
* Maintain case notes and patient records in good condition.
* Follow up on any investigations and results not received.
* Validate patient information on an ongoing basis.
Person Specification
Training & Qualifications
* Educated to secondary school level.
* GCSEs in English and Maths or equivalent (e.g., NVQ).
Experience
* Minimum of 6 months in an administrative or clerical role.
* Previous experience in a patient or customer care environment.
* Awareness of national policy concerning patient care.
* Data protection and confidentiality knowledge.
* Knowledge of HIS or patient administrative systems.
* Knowledge of hospital department clerical procedures.
* Previous clinic coordinator experience.
