Senior Community Mental Health Nurse in the Adult Community Mental Health Team
South West Shropshire
Due to an expansion of our team we are looking to recruit an experienced Community Mental Health Practitioner Band 6 to work 30 hours a week within the South West Shropshire Mental Health Team based in Ludlow.
The post offers the opportunity to work with a wide range of people living with severe and enduring mental illnesses utilising a recovery focused approach. The post will offer the opportunity to develop skills of assessment, develop collaborative plans of care, managing caseloads, liaison with families/carers, develop a wide range of interventions to aid recovery, and learn more about working with our Primary Care and Voluntary Sector colleagues in an integrated team.

The usual hours of work will be Tuesday to Friday 9am - 5pm but occasionally a degree of flexibility may be needed.
The candidate must be able to meet the travel requirements of the role in order to travel across the full geography of the catchment area.

Main duties of the job
We are looking to recruit a forward thinking, recovery focused dynamic mental health nurse with relevant knowledge and experience. The post holder needs to be able to demonstrate drive and motivation in providing evidence based interventions and support to individuals with severe and enduring mental health disorders.
Successful applicants will be able to function autonomously and in multidisciplinary teams. There will be an expectation that the post holder will work flexibly, possess excellent communication and interpersonal skills and effective clinical reasoning.
There will be an expectation that the post holder will hold a caseload of service users for whom they key work, provide supervision for Band 5 practitioners and support workers and work in collaboration with the multi-disciplinary team.
The environment can be fast paced and challenging, however, highly rewarding. Your role will involve working with other agencies, families, carers and local communities. Experience of working within a community mental health setting is desirable.
The post holder would have the opportunity to use their professional skills to contribute to the development of services and develop themselves personally and professionally in order to provide an evidenced based service. You will have the opportunity to work as part of the MDT working alongside Medical, Psychology, Nursing, Occupational Therapy and Support Worker colleagues.

Skills
* Skills and knowledge of a wide range of mental health conditions
* Previous experience in either an inpatient or community setting including a crisis team
* Demonstrates leadership ability or qualification
* Assessor/supervisor of student nurses
Additional Skills
* Knowledge of a wide range of mental health medications
* Phlebotomy skills or willingness to develop this skill

Salary: £37,338 to £44,962 a year (pro rata)
