Job summary An exciting opportunity has arisen for a part-time Care Coordinator / Administrator to join our multi-disciplinary team working at Plas Meddyg Surgery, APL PCN. You will play a key role in ensuring all patients receive the best possible care and service. The role plays a pivotal part in the vision of creating a wider community service and improving the quality of seamless coordinated care that enables MDTs to advance in their ways of working and ultimately contribute to the effectiveness of patient care provided. The Care Coordinator supports the Practice and other key stakeholders in coordinating all activity including access to services, advice and information, and ensuring health and care planning is timely, efficient and patient-centered. Working closely with other key members of the MDT building trusting relationships and enhancing collaborative working. The role is offered on a part time basis. Main duties of the job The Care Coordinator / Administator will work with GPs and other primary care professionals within the PCN to identify and manage a caseload of patients. Your main responsibilities will be assisting the surgery by undertaking a range of administrative duties to support patients with their health and care requirements. Provide a caring and efficient service to patients and the practice staff, thereby ensuring health and care planning is timely, efficient and patient-centred. Assisting with scanning, coding and work-flowing of patient documents. Monitor the surgery's generic email accounts and triage requests, providing advice and information, and updating patients medical records. Support the practice with oversight of the NHS's Quality and Outcomes Framework, ensuring patient cohorts, including at-risk groups, are recalled. Support the production and implementation of new working processes to optimise the quality of prescribing and patient care. Assist patients and the Medical Secretaries with queries regarding patient referrals and patient online access. Support the production of patient information as required including public health campaigns. Support the coordination and delivery of team meetings, internal and external inspections. Provide cover to various teams and this will require a good understanding of all of the roles.
