Responsibilities
Conduct assessments of clients' needs and develop personalized care plans.
Provide counseling and emotional support to clients facing personal challenges.
Collaborate with multidisciplinary teams to coordinate care and support services.
Advocate for clients’ rights and access to necessary resources.
Monitor client progress and adjust care plans as necessary.
Facilitate group sessions and workshops to promote client engagement and skills development.
Maintain accurate and confidential client records in compliance with regulations.
Requirements
Bachelor's or Master's degree in Social Work from an accredited institution.
Valid social work license and relevant certifications as required by the region.
Proven experience in social work or related field, preferably in a caregiving environment.
Strong knowledge of social work principles, practices, and ethical standards.
Excellent communication and interpersonal skills, able to build rapport with diverse populations.
Ability to work independently and collaboratively in a team-oriented setting.
Strong organizational skills and attention to detail for case documentation and management.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Annum: 08HC £35,235 - £39,513 (plus market supplement of £5,000 per year)
Location: Hereford, Herefordshire
