Principal Personal and Professional Accountabilities To act responsibly at all times ensuring that any actions do not bring the Hospice into disrepute. To promote care that is holistic in nature and is receptive to the individual needs of patients and their families. To carry out care and other assigned duties, under the support and supervision of a Registered Nurse. To demonstrate a personal commitment to the team working ethos and efficient functioning of the Hospice, contributing to the ongoing development of the Clinical team. To carry out all duties involving patients and their families in a courteous and respectful manner. To be receptive to the specific needs of patients and their relatives. To assist in the reception and admission of patients. To assist the patient as requested, and as determined by their individual nursing care plan. To support and provide assistance with all activities of daily living ensuring dignity and privacy are maintained at all times. To support the Nursing staff by engaging in roles associated with medication as directed, trained and deemed capable of fulfilling. To look for changes in the patients condition and report these to the Senior Nurse on duty. To demonstrate awareness of individual responsibilities in relation to the prevention of infection within the Hospice and to work at all times within the Hospices Infection Control policies and procedures. To ensure compliance with the hospice policies and procedures and have regard for the employees obligations under the Health and Safety at Work Act 1974. Communication and Working Relationships To ensure patients and visitors to the Hospice are made welcome and that all duties involving patients and their families are carried out in a courteous and respectful manner. To report all incidents/accidents when they occur to the Nurse in charge. To record the delivery of interventions as outlined in the individual patient care plan. To respect the confidential nature of all patient information. To promote clear and effective communication between all disciplines involved in patient care. To attend and participate in such meetings and events as may be reasonably required. Education and Training To participate in and contribute to own individual performance review, taking responsibility in the identification of personal training and development needs. To undergo such education, training and development as may be necessary to competently carry out the post. To support the Registered Nurses in the provision of patient and carer education. To attend the hospices twice yearly mandatory infection control training sessions at the Hospice. To attend Fire Safety Training as required and demonstrate awareness of the correct procedure to be undertaken in the event of a fire. To complete the Care Certificate qualification. To share best practice and specific knowledge with peers, enhancing confidence and promoting a diverse skill base. General Responsibilities - IPU To assist the patient as requested, and as determined by their individual nursing care plan. To record the delivery of interventions as outlined in the individual patient car plan. To assist in the serving of meals and drinks, providing additional refreshments for patients as requested. To assist patients in maintaining adequate nutrition and hydration where patients are unable to do this independently. To collect used crockery and cutlery etc. after patient meals, transporting these to the kitchen for washing if required. To assist in bed making activities and the safe removal of dirty linen to the laundry. To report all incidents/accidents when they occur to the Nurse in charge. To assist in maintaining the general tidiness of the clinical area. To monitor the use of all clinical equipment ensuring it is routinely and effectively cleaned, reporting any deficits or faults to the Line Manager. To assist in the arrangement of flowers, the changing of water, the removal of dead flowers and the watering of plants. To complete all relevant documentation to a good standard. To attend and participate in such meetings and events, as may be reasonably required.
