Job Description
Service Care Solutions has a new and exciting opportunity which has become available working as a Transitional Support Worker with Tameside Council.
Tameside Council is currently looking for someone who is happy to work 37 hours per week.
Key Responsibilities:
1. Establish face-to-face contact with young people and engage them with the service.
2. Conduct needs assessments and develop tailored plans to help young people achieve their aspirations.
3. Provide support for accessing education, training, employment, and community-based activities.
4. Liaise with external agencies to secure necessary support for young people, including housing, health services, and therapy.
5. Deliver group sessions and positive activities that promote personal growth and community involvement.
6. Uphold safeguarding principles and respond appropriately to any concerns.
7. Advocate for young people's voices, ensuring their views shape the services provided.
8. Maintain accurate records and manage caseloads effectively, adhering to departmental procedures.
9. Contribute to the ongoing development of the Staying Close Service.
About You:
We're looking for a compassionate, motivated individual who is passionate about supporting young people.
Essential Qualifications:
1. NVQ Level 3 in Care of Children and Young People, Adult Social Care, or equivalent.
Essential Skills and Experience:
1. Proven experience working with Cared-for Children and families.
2. Familiarity with Child Protection Procedures and relevant legislation for Care Leavers.
3. Strong IT, administrative, and organizational skills.
4. Ability to build meaningful relationships with young people and their families.
5. Knowledge of child development, attachment theory, and effective assessment techniques.
6. A commitment to anti-discriminatory and equal opportunity practices.
Desirable Skills and Experience:
1. Experience supporting families with substance abuse or domestic violence concerns.
2. Knowledge of parenting capacity assessments and evidence-based practices.
3. Understanding of care planning and court processes.
4. Full driving licence and access to a car for business use.
