We are seeking a dedicated and compassionate Care Manager to oversee the delivery of high-quality care services within our care home. The ideal candidate will possess strong communication skills in English, a valid driving licence, and a solid understanding of IT systems to effectively manage care plans and coordinate with staff, residents, and families. This role is pivotal in ensuring that our residents receive personalised care tailored to their individual needs. Responsibilities Develop, implement, and review individual care plans in collaboration with residents, families, and healthcare professionals. Ensure compliance with regulatory standards and best practices in care delivery. Supervise and support care staff, providing training and guidance as necessary. Communicate effectively with residents and their families to address concerns and provide updates on care progress. Maintain accurate records of care provided and ensure all documentation is up-to-date. Conduct regular assessments of resident needs and adjust care plans accordingly. Foster a positive environment that promotes the dignity and independence of residents. Collaborate with external agencies and healthcare providers to coordinate comprehensive care services. Requirements Proven experience in a similar role within a care home or home care setting is preferred. Strong communication skills in English, both verbal and written. Ability to drive and travel as required for the role. Proficient in IT systems for managing care plans and documentation. Knowledge of relevant legislation and best practices in the field of health and social care. A compassionate approach with a commitment to enhancing the quality of life for residents. Excellent organisational skills with the ability to manage multiple priorities effectively.
