As a core member of the team, you will also be responsible for your own caseload, providing ongoing assessment, care co-ordination as a Key Worker and delivering evidence based intervention in the community. You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work flexibly and collaboratively with patients, families/ carers and a range of statutory organisations.
You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team Manager and Advanced Practitioners.

If so, we have a post available for a permanent full time Band 5 Mental Health Practitioner at Stockton Community Hub, working in our Community Intervention Health Team.
You will be an active member of our multi-disciplinary team (MDT), working as a Key Worker, with a caseload which will be challenging at times, as well as rewarding. We will offer you the opportunity to work with a highly skilled and experienced MDT providing support, development opportunities and supervision for you, as well as excellent standards of care for our service users and carers.
We are an established team covering Stockton working in peoples own homes and other care environments.
Please note that the standard working hours for this role are Monday to Friday 9am to 5pm
