We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.
We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end of life care and outpatient and diagnostic imaging services.
We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

The postholder:-
· To contribute to a culture that recognises and works in partnership with patients and their families, involving them in all decisions.
· Receive patients and their relatives in a calm, courteous and reassuring manner, participate in providing information and support
· Provides early intervention to ensure the safe discharge of patients, working in collaboration with the medical team and community services with the aim of avoiding readmission.
· Give and receive regular reports on progress of patient, immediately acting on any changes in the patient’s condition, and reporting to the relevant health care professional. Maintain accurate records of observation, treatment and patient care
· Participate in the checking and administration of prescribed drugs (including controlled drugs) and other substances in accordance with Trust policy
· To have a willingness to take on enhanced/extended roles i.e. IV care of central lines and other clinical roles linked to the department.
· To act as an advocate for the patient and the family, including promoting and protecting their interests, rights equality and inclusion and privacy.
· Always uphold confidentiality due to patients and relatives and in all situations according to Trust policy
· To act as a ‘Link Nurse’ within a defined speciality/ interest area.
· To be involved in trust Initiatives to improve patient safety and family integrated care
· Ensure that all equipment is in good working order and ready for immediate use. Appropriately reporting any defective equipment or provision of supplies

· To work within the NMC code of professional practice, and within policies, procedures and guidelines of Calderdale & Huddersfield Foundation Trust.
· To participate in regular staff meetings.
· To be flexible to work within areas or departments to meet the demands of changing service needs.
· To provide mentorship and preceptorship to newly qualified staff and students, new starters and students encouraging an effective learning environment. To support the ward sister in providing a suitable and effective and inducive learning environment
· To take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements.
· To undertake the neonatal induction programme leading to Qualified In Speciality (QIS) with an agreed period within recruitment
· To communicate with the nurse in charge and collect data in order to participate in the smooth running and organisation of the department.
· To support junior and senior colleagues in their roles and be resilient to manage change.
· To support the ward sister in risk assessment and minimisation. Ensuring incidents and near misses are reported.
· To manage verbal complaints, inform ward sister and refer on when unable to resolve.
· To participate in the investigation of incidents/complaints as required.
· To establish and maintain effective communication, both verbal and written and relationships with multi-disciplinary team.
· To recognise, prevent where possible, and manage areas of conflict, reporting to ward sister if unable to resolve.
· Participate in research programmes utilising agreed research findings to enhance the patient and their family’s experience.
· Contribute to and participate in programmes of Audit, including utilisation of results in practice.
