Job Summary
To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the GMS contract. Furthermore, the post-holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team.
Duties and Responsibilities
1. Provide a full range of medical services as defined in the core GMS contract, additional and enhanced services where appropriate.
2. Provide other general medical services to meet identified patient need.
3. Make professional, autonomous decisions in relation to presenting problems whether self-referred or referred from other healthcare workers within the organisation.
4. Assess the healthcare needs of patients with undifferentiated and undiagnosed problems.
5. Screen patients for disease risk factors and early signs of illness.
6. Develop care plans for health in consultation with patients and in line with current practice; disease management protocols, provide counselling and health education.
7. Work within current CCG policy for prescribing.
8. Be available and accessible to patients at all times as agreed.
9. Maximise the effective use of current clinical computer systems.
10. Record clear and contemporaneous consultation notes to an agreed standard.
11. Compile and issue computer generated acute and repeat prescriptions (avoiding hand written prescriptions whenever possible).
12. Undertake all duties and responsibilities associated with a GP working within primary care.
13. Take part in, and lead on, areas including clinical auditing and data collection.
14. Support under-graduate student training in the surgery.
15. Support GP registrar training in practice.
16. Attend team meetings and other clinical meetings as necessary.
17. Provide short term emergency cover for unplanned absence of colleagues.
18. Provide emergency on call cover for the practice.
Service Development
Be involved in developing primary care services to meet the needs of the practice population. Working in close collaboration with the practice team to ensure that access targets are reached. Support health development and ensure health promotion work is undertaken to allow the practice population to make informed choices.
Managerial
Aware of, and compliance with, all relevant organisation policies and procedures, e.g. prescribing, confidentiality, data protection, health and safety and infection control. Apply these to all aspects of ones own work. Undertake all mandatory training as determined by the practice. Committed to life-long learning and audit to ensure evidence-based practice.
Quality
Adhere to GMC Good Medical practice. Maintain accurate and timely medical records. Adhere to National and local guidelines (e.g NICE). Engage with local quality frameworks e.g. QoF, Quality Improvement, Prescribing Incentive Schemes. Reflect on personal practice through SEA, NHS appraisal, patient complaints.
Confidentiality
All patients have the right to expect that all practice staff will respect their privacy and act appropriately at all times. In the performance of duties, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. All such information from any source is to be regarded as strictly confidential.
Communication
The post-holder must recognise the importance of effective communication within the team and will strive to communicate effectively with other team members, patients, carers and recognise people's needs to alternative methods of communication and respond accordingly.

Personal and Professional Development
Maintain continued education through attendance at courses and/or study days as necessary and as identified within clinical and organisation appraisals (Personal Development Plan). Ensure appropriate professional registration and licensing are met through continued professional development.
Collaborative Working
All staff are to recognise the significance of collaborative working. Teamwork is essential in multidisciplinary environments. Effective communication is essential and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate manner.
Security
The security of the practice is the responsibility of all personnel. Staff must ensure they remain vigilant at all times and report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door locks to anyone and are to ensure that restricted areas remain effectively secured.
