Key Responsibilities • Provide autonomous clinical care to patients of all ages, managing undifferentiated conditions across various settings, including: • Remote telephone triage and consultations – from base location. • Face-to-face assessments at base locations. • Home visits for patients requiring urgent care. • Conduct comprehensive history-taking, clinical examination, and assessment to diagnose and manage patients effectively. • Use advanced clinical reasoning to develop safe, patient-centred management plans, including appropriate prescribing, referrals, and safety netting. • Communicate effectively within a multidisciplinary team, ensuring collaborative care delivery across varying team compositions. • Demonstrate proficiency in utilizing healthcare technology and patient records to inform clinical decision-making. • Identify and manage patients presenting with complex needs, including acute illness, chronic conditions, mental health issues, and paediatric concerns (including children under 2 years of age). • Support patients with safety-netting advice, ensuring robust follow-up plans where required. • Engage in professional development activities, including regular audits, peer support forums, and educational sessions. • Contribute to service improvement initiatives and uphold the highest standards of care delivery. Person Specification Qualifications - Current registration with a relevant professional body (e.g., NMC, HCPC). - Non-medical prescribing qualification - Postgraduate Qualification in Advanced Practice - Advanced Clinical Practitioner MSc qualification (Level 7) working towards if not in possession. Experience - Demonstrable experience in autonomous practice, diagnosing and managing patients independently – Relevant experience in primary care, urgent care, ED, or other first-contact settings. - Experience in mental health assessment and management. - Evidence of comprehensive history-taking, clinical examination, and decision-making skills. - Experience in paediatrics including children under the age of two years. - Verification of death (willingness to undertake training). Skills and Attributes - Strong clinical reasoning and problem-solving abilities. - Excellent communication and interpersonal skills, adaptable to diverse team environments. - Ability to work effectively under pressure while maintaining high standards of care. - Knowledge of referral pathways and local healthcare systems. Desirable Familiarity with Adastra. Previous Experience as an ACP Experience in mentoring or supporting junior colleagues. Experience with telephone triage & remote consultations. Verification of death Advanced Clinical Practitioner MSc qualification (Level 7) completed. Qualification in leadership or service improvement. Evidence of continuous professional development.
