Job summary The Intensive Support Team (IST) has an exciting opportunity for two skilled, passionate, and dynamic Learning Disability or Mental Health Nurses to work creatively to make a real difference to the lives of Adults with a Learning Disability living in Derbyshire. We are looking for two individuals who share our strong value-based approach and have an understanding of the support needs of people with a Learning Disability and Autism, who are at risk of an acute hospital admission or placement breakdown. We are hoping to recruit two Band 6 Lead Nurses One full time position working 37.5 hours per week One part time position working 18.75hours per week Hours of working are flexible and we are happy to discuss further if required and can be negotiated with the right candidates. Both positions on a permanent contract and will be based at Ash Green LD Centre, Chesterfield, covering the North Derbyshire geographical area. Please be aware that the Intensive Support Team cover 7 days a week, 52 weeks a year; between the hours of 8.00am and 8.00pm. A variety of shift patterns are available, including 8.00am-4.00pm, 12.00noon-8.00pm and long days. Main duties of the job The IST is part of the multi-disciplinary Derbyshire Adult Neurodevelopmental Services. We work closely with a variety of stakeholders - Individuals, their families and carers, colleagues in Adult Care, Primary Care and Specialist Care Providers. We focus is on the individual and aim to provide responsive, intensive and personalised care, to avoid inappropriate hospital admissions. We work to maintain individuals in their own home and engaged in their own community, and follow the aims of the NHS 'Building the Right Support' Action Plan.
