Band 6 RMN - Community Mental Health Practitioner - York - £26 per hour
We are currently seeking a skilled and experienced Band 6 Registered Mental Health Nurse (RMN) to join a Community Mental Health team based in York. This is an excellent opportunity to provide essential support to individuals in the community, working within a multidisciplinary team to deliver high-quality mental health care.
About the Role
As a Community Mental Health Practitioner, you will be responsible for delivering mental health assessments, care planning, and interventions to individuals with complex mental health needs. You will work closely with service users, families, and other healthcare professionals to provide person-centred care that promotes recovery and independence. This role is fully on-site and can be undertaken on a full-time or part-time basis.
Key Responsibilities:
1. Conduct comprehensive mental health assessments and develop tailored care plans.
2. Provide evidence-based interventions to individuals with complex mental health needs.
3. Work collaboratively with multidisciplinary teams, including social workers, GPs, and therapists.
4. Carry out home visits and community-based support, ensuring person-centred care.
5. Maintain accurate clinical records and adhere to professional and legal standards.
Requirements:
1. Registered Mental Health Nurse (RMN) - Band 6
2. Previous community mental health experience is essential
3. Valid NMC registration
4. Full UK driving licence and access to a vehicle is required
5. Strong risk assessment and crisis intervention skills
6. Excellent communication and teamwork abilities
Working Hours & Pay Rate:
1. Monday to Friday: 9:00 AM - 5:00 PM
2. Pay Rate: £26 per hour
