The Quality and Patient Safety Team is looking for a committed and innovative individual with experience across all areas of quality and safety to join our team as aPatient Safety Incident Investigator. This role is a key part of our Patient Safety Investigation Team, which was established as part of our implementation of the new Patient Safety Incident Response Framework.

effective care to our patients. We believe PSIRF offers a real opportunity to improve the way we investigate and learn from incidents, and this
post is a key role in making this a reality.
The Patient Safety Incident Investigator will take the lead in investigating and reviewing incidents on behalf of the Trust, and will support patients, families, and staff through the process of an investigation. The post holder will conduct thorough investigations into patient safety incidents and other incidents, as necessary, using dedicated learning responses and ensuring Being Open principles are applied. When engaging with those affected, the purpose is to provide explanation and support through what can be a difficult process. This role will include dealing with and providing support to newly bereaved individuals some of whom will be deeply distressed.
