The role at Glastonbury Surgery requires the following responsibilities: Reviewing discharge summaries and letters from medical professionals and taking necessary action, such as amending repeat medication and arranging blood tests. Operate a telephone appointment system to carry out medicines optimisation tasks, including effective medicine administration, supporting medication reviews and reconciliation. This will involve the utilisation of remote consultation skills to work in partnership with patients to ensure they use their medication effectively. Assist and take responsibility for, under the guidance of the Clinical Pharmacist, the delivery of medicines and patient safety audits e.g. Eclipse alerts and CQC searches. Using the results of the search the Pharmacy Technician will progress the planning of any required monitoring such as blood tests by inviting patients or arranging district nurse visits for housebound patients. Oversee and support the administration of prescriptions, stepping in to support capacity and achieve the timely management of prescription requests. Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients. To take a proactive approach to the safe handling of prescriptions including assessment of risk and making recommendations for improvement/update protocols. Support the practice reception team in streaming general prescription requests, so as to allow GPs and clinical pharmacists to review the more clinically complex requests. Be accessible to the wider non-clinical team to assist with daily ad-hoc medication queries and requests. Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. Support the practice to deliver on QOF and locally commissioned enhanced services where appropriate. Develop relationships with other pharmacy professionals and members of the multi-disciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care and mental health.
