It's an exciting time in the recently formed Hampshire and Isle of Wight healthcare with with new and exciting Medicines Management posts, these include a hospital at home frailty Virtual Ward (H@H VW), pharmacist and a Pharmacy Technician working in partnership with the consultant practitioner and foundation consultant practitioner. The H@H VW supports patients who would otherwise be in hospital to receive the acute care and treatment in their own home, including intravenous therapies. The Urgent Community Response Team (UCR) is a rapid assessment, diagnostic and treatment service to reduce unscheduled admissions for people with complex health needs, exacerbation of long-term conditions and frailty. The service provides responds to crisis, recovery support, self-management, and adopts a collaborative approach with patients. The post holder will be expected to work between two teams across Mid and North Hampshire, with team bases in Herriard, located in-between Basingstoke and Alton,
and Colden Common in Winchester. A hybrid approach to working will be considered for the right person. The post holder will be required to work autonomously to: To deliver a high-quality Medicines Optimisation Service to high-risk patients and ensure an integrated approach to the management of medicines and continuity of pharmaceutical care for patients. To provide professional expertise, advice, support and training on policies and practice to health and social care professionals in the community., Conduct clinical medication reviews with patients in their own homes to ensure patients medicines are optimised for them to gain the maximum health benefits by utilising relevant and current evidence-base and local/national guidance. Ensuring the treatment is individualised taking into account the patient's ability/willingness to adhere with the prescribed medicines regimen. Make prescribing recommendations as per local and national guidance to the H@H VW and UCR team. To prescribe and deprescribe within their competence for patients to improve patient care. Provide expert pharmaceutical information and advice to the H@H VW and UCR multi-disciplinary teams and where applicable to others involved in a patient's care. To work closely with the services' Pharmacy Technician/s and support with their professional supervision and development. Focus on outcomes delivering improved benefits and value to patients from their use of medicines, appliances, and devices. To be responsible for
overseeing medicines storage and security. To be responsible for medicines supply (e.g. stock lists) ensuring appropriate quantities are stored in accordance with the changing clinical needs of each clinical environment to minimise waste and reduce risk. Contribute to agreed meetings within and outside the organisation and represent pharmacy as appropriate. Respond to unpredictable work patterns and urgent requests for help by phone or email within a reasonable time frame.
