About the role
You will assess and review the needs of clients who are being referred for, or already in receipt of, care and support services, and undertake and produce high quality assessments, reviews, and care and support plans which focus on both achieving independence for and desired outcomes of the individual.
You will work in partnership with other professionals and organisations to support our residents effectively and holistically. Social Work Assistants will endorse and act in accordance with the principles of personalisation, ensuring that care and support plans are person-centred, and as far as is safely possible, you will put the people with whom you work with in control of their lives.
On-the-go:
Our ‘on-the-go’ roles mean you’ll usually start and finish your day from a base, moving around between different locations throughout the day. You won’t need a fixed desk to work from, but you’ll have access to our network where you can keep up to date with admin and the latest staff updates.
About you
We are looking for Social Work Assistants who are passionate, supportive, motivated, and confident when supporting and communicating with people - either on the telephone or face to face.
Your work will include carrying out better lives assessments, creating person-centred and strengths-based care plans, ensuring delivery of services, and reviewing outcomes with the individual, their personal networks and support providers.
You will also demonstrate emotional resilience and intelligence - which is the ability to maintain focus and to ‘keep going’ despite setbacks and stressful situations.
You must have a full driving license with access to a vehicle.

About the Service Area
The Home Independence service supports individuals to regain their independence following illness, disability, or recent stay in hospital. We work with clients in their homes, identifying whether they now have difficulties with self-care or performing daily activities. We provide a goal-based assessment for each client and alongside them to support the achievement of their goals, using technology-enabled care devices and occupational therapy aids to enable them to achieve independence.
The team works closely with healthcare professionals, carers, clients and their families to develop suitable packages of support, designed to enhance client independence and meet their needs. We supply advice on specialist equipment options, monitor client progress and make necessary adjustments if required. If you enjoy working in a fast-paced environment alongside individuals who care about the wellbeing of our residents, the Home Independence team may be for you.
